Hearts & Hands Disaster Recovery (HHDR) provides Disaster Case Management to the residents of the 24 disaster declared counties of South Carolina.  If you are in need of case management for the October 2015 Flood please call 803-851-5551 or email us at info@hhd.ngo. Intake for Disaster Case Management services for this event will close April 30, 2017.

Lexington County, SC CDBG-DR

If you are a resident of Lexington County and sustained damages from the October 2015 Flood and would like to apply for home repairs or the Buyout program, residents may either call (803-851-5551), email lexingtondr@hhdr.ngo  or apply in person at the HHDR office at 401 Western Lane Suite 8B Irmo, SC to start the application process. Please note the list of required application documents is listed below. Applications are not complete until all of these documents have been submitted to the HHDR Intake Specialist. The deadline for applying for this program is April 28, 2017.

Required Information & Documents for Application

  • HHDR Consent form
  • Photo ID for all members of household over the age of 18
    • State ID, Driver’s License, or Military ID
  • A copy of bank statements for the last 6 months
    • If self employed statements for the last 2 years are required
  • Pay stubs for the last 2 months for all household members over the age of 18 who are not full time students
  • Tax Returns from the last 2 years that have been filed (1040) for all household members 18 and older including all attachments and schedules
    • Returns must reflect the number of children living in the home
  • Current SSI, Disability, Retirement Statements for last 2 months, if applicable
  • Signed 4506-T Form for Non Tax Filers
  • Household Income Certification
  • Current mortgage statement
  • Property Tax Documents 2015 & 2016 tax receipts
  • Homeowners and Flood insurance declaration page if applicable
  • Insurance Claims: Flood Insurance and/or Homeowners
  • FEMA award/denial letter
  • SBA award/denial, if applicable
  • Receipts for all repair/rebuild funds received
  • Duplication of Benefits calculation
  • Proof of ownership ie deed/title if name does not match property taxes
  • List and description of all prior work done to home
  • Property structure and type
